You find elsewhere in the book that her maiden name was Louise Jones. Whether you make a reference from the maiden name or not depends on usage; AACR2 chapter 26 instructs that headings and references should be based on usage in the author's works or in reference works.

You find an old LC bib record with the heading: Butler, Harrison L. The statement of responsibility reads: by Harrison L. Butler.
You must use the LC heading when the usage matches the AACR2 form of the name, as in this case.
You would form the heading as follows:
  100 10 Butler, Harrison L.
A reference is not appropriate since the primary elements of the reference would be the same as the heading.

You would form the heading and cross references as follows:
